US, Afghanistan start talks on status of US troops

KABUL, Afghanistan (AP) The United States and Afghanistan are starting negotiations on an agreement that will shape America's military presence in the country after the withdrawal of most foreign combat troops at the end of 2014.

Representatives from the two countries began talks on a bilateral security accord on Thursday in Kabul.

They will grapple with potentially divisive issues about the future U.S. military footprint in Afghanistan and whether U.S. troops can be prosecuted under Afghan law an issue that sank America's security deal with Iraq last year.

After Thursday, the next round is expected in December, although decisions on the most contentious issues could be put off until later.

The lead negotiators are James Warlick, the U.S. deputy special representative for Afghanistan and Pakistan, and Eklil Hakimi, Afghanistan's ambassador in Washington.

Nate Silver Turns His Eye To the American League

As a baseball fan, I have read literally dozens of articles and hundreds of message boards rants on this subject. If you're interested, a little wading through Baseball Think Factory [baseballthinkfactory.org] will allow you to relive the endless re-hashings at your leisure. More generally, this sort of statistical talk is very common among a certain segment of baseball fandom, and is (as has been mentioned before) the milieu from which Nate Silver emerged.

What's interesting about this specific issue is that Cabrera vs. Trout has been painted as a traditionalists vs. stat-heads vote, but an argument for Trout can be made with no reference to advance statistics. It goes like this:

Trout's traditional "slash line" (batting average, on-base percentage, and slugging percentage) is very similar to Cabrera's. Cabrera hit for more power, but otherwise they are nearly equal. Trout's home ballpark is harder to hit in than Cabrera's. Trout led the league in stolen bases with 49, Cabrera had 4. Cabrera grounded into 28 double plays, most in the league. Trout grounded into 7. Cabrera is a poor defensive player, Trout is an outstanding defensive player. Trout's team even had a better record than Cabrera's, even though Cabrera's Tigers made the playoffs and Trout's Angels didn't.

Nothing in that argument requires anything more complicated than the division required to work out batting average and the like. The fact that Trout's candidacy has been painted as just the result of statistical mumbo jumbo is ridiculous.

(It should be pointed out that there is a lot of mumbo jumbo in baseball's defensive statistics. They are not at all mature yet, and are heavily influenced by very subjective inputs. This is part of why I prefer the non-statistical argument for Trout. When someone says that Trout's glove was worth 2.1 wins above a replacement player (the number given at Baseball Reference [baseball-reference.com] he is speaking with a false precision. Silver, it should be noted, doesn't fall into this trap, and I should say that Sean Foreman at Baseball Reference doesn't believe that his 2.1 win number is anything more than an educated guess.)

Egypt recalls envoy to Israel after Gaza strike

CAIRO (AP) ? Egypt's Islamist president recalled the country's ambassador to Israel to protest its strikes in the Gaza Strip on Wednesday after coming under mounting domestic criticism for not taking a strong enough stance against Israel.

The move may signal a shift in the way Cairo deals with Israel following last year's popular uprising that ushered in the country's first free presidential elections over the summer and a wave of protests since. Ousted President Hosni Mubarak, who built close ties with Egypt's neighbor, was accused by Egyptians for refusing to bend to popular pressure for tough stands against Israel.

Ironically, his successor President Mohammed Morsi, who hails from the Muslim Brotherhood, came under similar sharp criticism earlier this week for failing to speak out after seven Palestinians were killed in Israeli airstrikes over the weekend in retaliation for rocket attacks by Gaza militants. Secular parties and revolutionary groups on Monday held a vigil denouncing Morsi and protesting the Israeli airstrikes.

On Wednesday, Israel heightened its offensive by killing Ahmad Jabari, the commander of the Hamas military wing, in one of some 20 airstrikes on the Gaza Strip in the wake of further militant rocket attacks. Ten people were killed in the assault on Gaza, two of them young children.

This prompted a protest of around 200 people in downtown Cairo on Wednesday who demanded Morsi take a stand against the Israeli attack.

"Morsi where is your decision," they chanted. "Our leaders, enough with the silence. The people of Gaza are dying there."

Late Wednesday, presidential spokesman Yasser Ali announced on state TV that Morsi had recalled the Egyptian ambassador.

The Muslim Brotherhood is a vocal critic of Israel, and the Palestinian Hamas militant group that rules Gaza is an offshoot of the group. Since taking office as Egypt's first freely elected president in late June, Morsi has refused to meet or contact any Israeli officials and doesn't even mention Israel by name in official statements.

Still, while relations have cooled, Morsi has not brought radical change in Egypt's policy toward Israel. He has promised to abide by Egypt's 1979 peace deal with Israel and his government has continued contacts with Israel through its non-Brotherhood members.

Israel and Egypt have quietly cooperated over an Egyptian military operation in the Sinai Peninsula against Islamic militants who have been attacking Egyptian forces and launching attacks into Israel. Morsi's government also still tightly controls Palestinian movement through the Rafah border crossing with Gaza in the Sinai. Under Mubarak, Egypt's backing of a blockade of Gaza imposed by Israel after Hamas won elections and later overran the territory in 2007 was highly unpopular among Egyptians.

As an elected leader, however, Morsi faces heavy pressure to reflect more strongly the widespread anti-Israeli feelings among the Egyptian public.

"Before, in Mubarak's time, there was no response at all and they suffocated Gaza," said Hassan Saleh, a 25 year-old education ministry official who was among those protesting in Cairo on Wednesday. "All this must change. We should allow aid in, open the crossing and hospitals and pressure Israel."

The Brotherhood's political party, which Morsi headed before winning the presidency, warned in a statement Wednesday that "the occupation state must realize that the changes that took place in the region, especially in Egypt, will not let the Palestinian people fall at the mercy of the Israeli aggression as was the case before."

The party expressed condolences for al-Jabari's death and denounced the Israeli airstrike Wednesday as a "crime that requires a quick Arab and international response to stem these massacres." It said Israel wants to "drag the region toward instability." They called for protests Friday to denounce the military escalation.

Egypt's ambassador to Israel Atef Sayid al-Ahl told the semi-official Al-Akhbar newspaper he would arrive to Cairo on Thursday afternoon. Israeli Foreign Ministry spokesman Yigal Palmor said Israel had not received word of the recall and would not comment until it did.

Morsi also asked the Arab League's secretary-general to convene an emergency ministerial meeting in the wake of the Gaza violence, his spokesman said. The Arab League said it would hold the emergency session on Saturday. The Foreign Ministry delivered a letter to the Israeli Embassy in Egypt expressing its "strong condemnation" of the Israeli strikes.

Egyptian military intelligence officials said Wednesday four missiles were fired from northern Sinai into Israel just south of the Kerem Shalom border crossing. The officials said they could not confirm what type or rockets were fired or where they landed inside Israel.

Islamic militants are suspected of being behind the rocket fire, the officials said, speaking on condition of anonymity because they were not authorized to speak to the media.

Superstorm priorities for some include saving pets

NEW YORK (AP) ? Superstorm Sandy drove New York and New Jersey residents from their homes, destroyed belongings and forced them to find shelter for themselves ? and for their pets. Owners recounted tales of a dog swimming through flooded streets and extra food left behind for a tarantula no one was willing to take in.

In New York City and on Long Island, the ASPCA has rescued more than 300 animals and treated or provided supplies to about 13,000, working with government and private animal welfare agencies, said spokeswoman Emily Schneider.

City shelters took in about 400 animals along with their families in the first days after Sandy, Schneider said. There are now more than 100 in shelters with their owners, and a mobile animal medical clinic is cruising decimated neighborhoods in Queens' Rockaways and on Staten Island.

In New Jersey, the Humane Society deployed dozens of first responders using mobile units and boats to bring in about 60 displaced animals each day on the barrier islands hit by the storm.

The absence of animals that were lost or separated from their owners only adds to families' feelings of displacement and trauma, said members of emergency crews trying to rescue both.

Two weeks after Sandy made landfall on the Atlantic shore, followed a week later by a nor'easter, search-and-rescue teams were led by Animal Care & Control of NYC, a city-contracted nonprofit responding to hotline calls about pets in distress. Callers are owners forced to leave animals behind or unable to care for them, or people who see them wandering in hard-hit areas.

A Manhattan shelter takes in animals round the clock, hoping for owners to show up.

And social media teams scour the Internet for reports of lost pets, helping reunite them with owners.

Rescuing animals is mandatory under federal law, which requires local and state governments to include plans for pets in emergency procedures. Federal Emergency Management Agency funds go toward the welfare of animals in disaster zones.

New York City's human shelters are required to accept pets, and so are taxis and public transportation.

Some find it hard to understand why animals are a key concern in disasters engulfing human lives, but owners feel an attachment and responsibility to their pets, said Niki Dawson, director of disaster services for the Washington, D.C.-based Humane Society of the United States.

"There's such a strong bond between people and animals that people will put their lives at risk not to leave a pet behind," Dawson said. "So they stay, even when they're told to evacuate, and that puts first responders going back for them at risk."

More than 200 dogs, cats and other pets from a devastated area of New York's Long Island are being sheltered in the gymnasium of a community college. Many belong to owners in nearby shelters and hotels.

Celebrity chef Rachael Ray is donating $500,000 to the ASPCA to help pets and families struggling to rebound from Superstorm Sandy. She said her pet food brand, Nutrish, is also shipping 4 tons of wet and dry dog food for Sandy animals, and her Yum-o! organization is donating $100,000 to City Harvest and the Food Bank for New York City.

The ASPCA will use the money to lease a building to board Sandy animals till their owners can take care of them.

Schwartz and Scooter, two black felines, came from a home that was flooded with water 5 feet deep, dark and cold in Long Beach, a heavily damaged barrier island community.

On Kate and Warren Sherwood's block, five houses were burned to the ground.

"We barely got out of our house in time," Kate Sherwood said.

They took refuge at a hotel that didn't accept cats.

"We sneaked them in and put a 'do not disturb' sign on the door ? pretending we're on our honeymoon," said Warren Sherwood, 56, a systems analyst. "But after three days, they got restless and starting meowing."

The couple took them to the Nassau gym shelter, run by the North Shore Animal League America, the nation's largest no-kill rescue and adoption organization.

"We're ridiculously stressed out, we're freaked out," said Warren Sherwood. "But I'd do anything in the world for these people who are keeping our cats alive."

Also among the pets in the gym was Emma, a Manchester terrier that swam to safety through flooded streets in Freeport on Long Island, while its owners carried their cats above the water, plus some clothes they grabbed at the last minute.

"We lost our house. It's submerged," said Mark Swing, who fled with his girlfriend as the tides rose. "All we got out was our four cats and the dog, except for a few changes of clothes."

The 8-year-old terrier was "a little tired, but fine," said Swing, 48, a contractor who was in a Red Cross shelter.

Cats and dogs weren't the only pets rescued from the storms.

"We're finding chinchillas, guinea pigs, rabbits, reptiles, birds," said Dawson.

And then, there was the tarantula, left behind in the New York City borough of Staten Island, in a home inspectors deemed uninhabitable.

The spider belonged to a teenage boy who was also attached to his gerbils. He and his family were forced to evacuate.

His aunt in Brooklyn agreed to take in the gerbils, but no one wanted the hairy tarantula. The teen left it behind with lots of food ? in hopes the spider could be retrieved later.

Like their owners, many animals that survived won't go home anytime soon. They're being housed and fed thanks to the kindness of strangers.

Transport trailers distributed pet food and supplies like crates, leashes and litter from a warehouse in Queens set up days before Sandy descended, said Schneider. Tons of food is being trucked in, donated by Petsmart Charities, Iams, Del Monte Foods, Purina and the Petco Foundation.

It will be months before any estimates are available as to how many pets might have died or were lost during New York's double storms.

Dawson says she's seen people stuck in shelters, wearing donated clothing, with no idea when they'll go home. But when they turn to their dog or cat, "their faces light up."

And that, she says, is why animals matter amid a human disaster.

NY auction of Warhol works fetches over $17M

NEW YORK (AP) ? The first in a series of live and online auctions to raise money for the Andy Warhol Foundation for the Visual Arts in New York has collected over $17 million.

Monday's auction at Christie's featured works by Warhol ranging from prints to photographs, some of which have not been seen by the public.

The top sale was "Endangered Species: San Francisco Silverspot," a print that fetched over $1.2 million. Other highlights included "Jackie," a screen print and paper collage of Jacqueline Kennedy that sold for over $626,000. That's more than double its high estimate of $300,000.

The foundation says the money raised for its endowment from the sales would allow it to expand support of the visual arts, fulfilling Warhol's purpose in establishing it.

Online auctions will begin in February.

Faking It While Making It?

I?ve been listening to way too much Dan Savage lately. For those of you who aren?t aware of the Savage Love podcast, it?s a weekly show where people call in with questions about every facet of sexuality. There is a lot of talk about fetishes and polygamy and activities far beyond the realm of what we consider ?normal.? There are also a lot of questions about the most mundane and boring elements of human sexuality. This is a show for everyone. Well, everyone except children and my 95-year-old grandmother.

Savage is considered a controversial figure by some because of?among other things?his encouragement of polyamorous relationships, or what he calls ?monogamish? relationships. He argues that most people will stray, and even if they don?t, the temptation will be there. A monogamish relationship doesn?t just allow for extra partners, but gives sexual partners the freedom to discuss their fantasies and concerns openly and honestly. It?s the kind of relationship where agreements are made, contracts are figuratively drawn up, and both parties are consulted before any actions are taken.

It is a very adult idea of sexuality?and by ?adult? I don?t mean anything as titillating as porn. Having sex on the kitchen table can be sexy. Rationally discussing your sexual issues over coffee at the kitchen table usually isn?t. Dirty talk is hot. Talking about your therapist?s recommendations with your partner isn?t.

So, sorry to disappoint you. When I say that I?ve been listening to too much Savage lately, it has nothing to do with riding crops or clear heels or threesomes. If it was, I could be totally honest and happy to purchase some shiny new items from Smitten Kitten or Adam & Eve and call into work for the next week or so. It?s that open and honest thing that makes me want to wrap my body in a nun?s habit and a five-fold saffron robe.

Sexual honesty makes me feel filthier than a Jenna Jameson DVD. It?s why I spent most of my adult life avoiding it?faking it while I was making it. Down through the years, I?ve had a lot of Meryl Streep moments in the bedroom. I?ve acted interested when I?m not. I?ve acted disinterested when there is nothing more I?d rather do. I?ve feigned satisfaction and disappointment and transcendence and an overwhelming need to get a good night?s sleep. I?ve pretended to feel desirable while stealthily removing my Spanx. I?ve pretended to desire my partners even when they reeked of Jacked Smoky Chipotle BBQ Doritos and Miller Lite.

I?m sure I, and billions of other people, could offer up our sexual relationships for the Academy?s consideration. Our childhoods are filled with fairy tales and mythology and movies and pop songs. We spend our teenage years painfully realizing that all those stories either aren?t true, or just aren?t true for us. We become disenchanted, angry, vulnerable. We listen to a lot of music and experiment with a range of substances to deal with that reality. Then we grow up and have to date. By adulthood, we are wounded veterans with bloody hearts and haunted brains and a declining ability to tolerate alcohol.

At least, we should be. There are, out there, adults who believe that Sleepless in Seattle was a documentary. That you complete me is an endearing and positive statement. That you can meet someone once and put a deposit down on a U-Haul. I?ve been on unsuspecting internet dates with some of them and marveled at their hopefulness, otherwise known as codependency issues. Sex usually wasn?t a concern with these hapless fellows. Getting through a beer without giving them a wake-up call was.

But then you get a live one?a person who does get you but doesn?t want to be 50 percent of you. Someone with complimentary wounds and a well-stocked first aid kit. You begin letting down your well-reinforced guard and, almost accidentally, being your true self. It feels strange after all those years of learning what other people want and being that, to just being.

Some people never get there. I?m terrified I might be one of them.

As I?ve already admitted, it isn?t that I have that much to reveal when it comes to sex. I have a past, yes. I?m a 34-year-old woman who has been essentially single and sexually active her entire adult life. There have been some cringe-worthy, dangerous, salacious, very satisfying moments with more than a few partners. And as a woman, I?ve been violated and mistreated by men during several hellish periods of my life. I know very few women who haven?t been, though. So many women I know harbor some incident from their past in the deepest part of their souls. That place of the unforgiven. That mute place.

I?ve always believed that silence is, indeed, golden when it comes to sex. I?ve been able to ask for what I want and to prevent what I don?t want, but that?s because I feel it?s my feminist duty to educate and enlighten men. That voice comes from the confident, politically aware part of my libido that demands that I enjoy sex because my foremothers didn?t have to the right to.

This orgasm?s for you, Grandma!

But then there?s that amphibian me who is dismissive of intellectual thought and self-help books. It?s the part of me that fears men and fears myself. Some of my fear of men is founded, but in my current relationship, the fight or flight response only leads to another night of Parks and Rec re-runs and some low-sodium popcorn. I?m running away from him, running toward any kind of distraction to avoid talking about it or having it.

There are all my fears of sexual inadequacy; the montages of bad porn and Angelina Jolie and bony hipbones that plague my mind are Hollywood-slick. Several years ago, I stopped telling men that I was a yoga instructor because they expected Kama Sutra. They expected handstand blowjobs and chakra explosions. Just being honest.

There is the fear that I?m not enough of a freak, and the fear that I?m far too much of one. And most of all, there?s the fear that I have to admit all of this to another person. That we are so close, that the smoke and mirrors that made dating so mysterious and fun, make our relationship distant and confusing.

The truth about sex is that it can be just sex and be fulfilling. No harps or roses or promises of eternal love are required. To silence yourself is to remove the pleasure and spontaneity in favor of the mask and bad dialogue.

(AP) ? A trans-Atlantic collaboration between the Royal Shakespeare Company in England, Miami's GableStage and The Public Theater in New York will present a radically fresh version of "Antony and Cleopatra" set on the eve of the Haitian Revolution.

The new staging, which is edited and directed by Tarell Alvin McCraney, will premiere at the Royal Shakespeare Company's Stratford-upon-Avon home in November 2013 before coming to the United States to play at GableStage in January 2014 followed by The Public Theater in late January 2014.

Setting the Shakespeare play in the 1700s allows McCraney, an artistic associate at the Royal Shakespeare Company and an ensemble member of the Steppenwolf Theater Company, to explore colonialism and racial politics of the New World.

The Lumia 820 has been tucked away behind the shadow of its bigger brother ever since it was revealed, but if any phone represents the Windows Phone 8 vision -- different phones for different people -- it's this next-generation Lumia. Avoiding the unibody build of the rest of the family, this phone has an outer shell that's removable and can work with a range of cases: glossy, not-glossy, ruggedized and capable of wireless charging. It's a nice option, one that offers a taste of hardware customization before you've even started swiping around the Live Tiles and customizing the color schemes.

Under the lid, there's a removable 1,650mAh battery, with access to expandable microSD storage. Other specs include 1GB of RAM, the same 1.5GHz Snapdragon S4 processor used in the Lumia 920 and a 4.3-inch OLED 800 x 480 display, albeit with the same glove-courting super-sensitivity found on that other new Lumia. On AT&T, it'll be $50 upfront, half the outlay for the 920, while in the UK, it's priced SIM-free at £360 in the UK, again compared to £445 for the bigger model. At this reduced price, you won't get to sample Nokia's optical image stabilization, but you'll still get a Carl Zeiss lens paired with an 8-megapixel sensor. So there's some understandable drawbacks compared to Nokia's statement phone and its two greatest strengths, but how does Windows Phone 8 fare on a cheaper handset? Big Sandy clean-up: Volunteers dive in to help

"We are cold and we are tired and we want out power now," said one Long Island resident. She said LIPA, the utility company, hasn't worked quickly enough. NBC's Michelle Franzen reports.

Hundreds of volunteers got on buses at City Hall on Saturday morning to make their way out to areas still struggling from Sandy's devastation.

Mayor Michael Bloomberg dedicated Saturday a day of service as volunteers spent the day distributing aid and helping with storm recovery efforts.

"It's time for us to help everybody back," said New Orleans mayor Mitch Landrieu. After Katrina, firefighters from New York and New Jersey lent a helping hand, so residents of New Orleans are sending both necessities and messages of hope to those impacted by Sandy. NBC's Janet Shamlian reports.

Teams cleaned up the beach and boardwalk in Coney Island, handed out food and water and helped with the cleanup on Staten Island, and hosted a clothing drive and children's carnival in Rockaway.


In a church parking lot on New Dorp, Staten Island, Walt Street cooked food for storm victims. Street, who had driven 30 hours from New Orleans, Louisiana to pitch in with relief efforts, said the scenes he was seeing on Staten Island reminded him of his hometown after Hurricane Katrina.

Defend New York: Experts on protecting against storms

On Long Island, about 300 people staged a rally in front of the Long Island Power Authority's office in Hicksville Saturday. Approximately 130,000 homes and businesses were still without power on Long Island.

"We are sitting in a cold house. No one comes by,'' said John Mangin of Levittown. "There should be criminal charges against the CEO and the executive board of LIPA for failure to do their jobs.''

Coney Island's New York Aquarium is filled with wonders from the sea, but during Hurricane Sandy the ocean devastated it. NBC's Mara Schiavocampo reports.

Gov. Andrew Cuomo has called for an investigation of the region's utilities, criticizing them as unprepared and badly managed. On Friday, two congressmen from Long Island called for the federal government to help LIPA restore electricity.

"It's a totally disorganized effort, and LIPA unfortunately seems to have lost control of the situation and that's why you see so many people becoming so angry,'' Rep. Peter King said Saturday.

LIPA chief operating officer Michael Hervey said they were aware customers haven't gotten the information they've needed from the utility, partly because of an outdated information technology system they're in the process of updating.

"I certainly feel the frustration of customers whose power remains out. Our hearts go out to them," Hervey said.

But he said workers are repairing unprecedented storm damage as fast as they can. About 6,400 linemen and 3,700 tree trimmers are at work, compared with 200 linemen on a normal day.

In New Jersey, Gov. Chris Christie said he expected power to be restored across the state by Saturday night. As of Saturday evening, Jersey Central Power & Light had nearly 62,000 customers overall without power.? Public Service Electric & Gas said 5,580 customers remain without power from the hurricane. Another 17,148 customers -- including those who lost electricity during the nor'easter and in recent days -- were also awaiting restoration. About 100 Orange & Rockland customers had no power, while Atlantic City Electric had roughly 2,480 outages.

Con Edison said just under 20,000 customers were without power, with just under 9,000 of those outages in Westchester county. Virtually all utility customers in Connecticut have had their power restored.

Cuomo announced Saturday that New Yorkers who lost their job or income because of Sandy would now have until February 4, 2013 to apply for federal disaster unemployment assistance. Claimants also now have up to 90 days to submit proof of employment or self-employment and will be provided an IRS link to speed up the retrieval of lost or destroyed tax documents.

New Jersey moved about 200 people from tents at the Monmouth Park Racetrack into the track's four story grandstand building. The storm victims were initially staying in a? camp designed for out of state utility workers, but the state came under fire after photos emerged showing people huddled together in snow covered tents.

The Statue of Liberty was illuminated for the first time since Sandy, after the storm caused significant damage to statue and to Ellis Island. The dock at Liberty Island that welcomes visitors was so badly damaged it might have to be rebuilt, officials said. The statue's interior had just reopened to the public before the storm hit. No word yet on when the statue will open to the public again.

Cuomo announced the MTA will resume service on the A subway line to Howard Beach Sunday. A shuttle bus will run from Howard Beach to Mott Avenue in Far Rockaway until the heavily damaged north channel bridge can be repaired.

Rockaway will also get temporary weekday ferry service into Manhattan starting Monday. Mayor Bloomberg announced the trips will cost two dollars each way. The ferries will make five morning rush hour trips to Pier 11 in lower Manhattan starting at 5:45 a.m., and five evening trips back to Rockaway. Free transfers from Pier 11 to the East 34th street midtown ferry terminal will be made available.

President Barack Obama will travel to the New York area next week to view the recovery from Sandy and thank first responders, the White House announced Friday.

Obama visited the Jersey Shore last week after Sandy pummeled the tri-state, telling storm victims: "We are here for you and we will not forget. We will follow up to make sure you get all the help you need to rebuild."

At the time, Bloomberg said he told Obama's chief of staff that the city "would love to have him, but we've got lots of things to do."

On Friday the mayor said "we've made significant progress since then and we're happy to have him."

Cuomo also announced the creation of the Empire State Relief Fund, intended to help residents repair and return to homes flooded and damaged when the storm hit last week. He says the best temporary housing is permanent housing and it's cheaper in the long run for taxpayers.

Bloomberg also announced a Sandy repair program, called NYC Rapid Repairs, aimed at getting as many people as possible back into their homes by year's end. Starting Tuesday people with storm-damaged homes can go to a city restoration center, get a Federal Emergency Management Agency number and sign up. Then a contractor will go to the person's home, assess the damage and "within a short time, the work will be done," Bloomberg said.

Understanding Pharmacovigilance System & Processes

According to WHO Pharmacovigilance is the science and activities relating to the detection, assessment, understanding and prevention of adverse effects or any other drug-related problem. In other words the goal of pharmacovigilance is getting the best outcome of therapy by patient care and surveillance. Good pharmacovigilance will identify the risks and risk factors related to adverse drug reactions (ADRs) during the complete product life cycle. This information allows for intelligent, evidence-based prescription and has the potential for preventing ADRs.

The importance of Pharmacovigilance
The primary aim is to enhance patient safety and to provide reliable and balanced information for the effective use of medicines. The secondary aim is communication of new safety ?ndings to the national and international regulatory authority in a timely manner. Pharmacovigilance programmes promote the education and training in this field and its effective communication to the public. Pharmacovigilance identify adverse events and their rates with the possible risk factors. It assesses causality with respect to clinical relevance, frequency and distribution of ADRs, which helps to understand the benefit, harm, effectiveness and risk of medicines.

Good Pharmacovigilance Practice
The concept of pharmacovigilance is normally not well understood, either by health professionals, patients or the general population. Development of guidelines and standards are required for a coherent and good pharmacovigilance system, which describe the practical details of the intended information Now.

Good pharmacovigilance systems should have the capability for receiving and processing of reports with veri?cation, interpretation and case causality assessment. It should have databases and procedures for data analysis, review and signal detection. An effective coordination between health care settings, regulatory and public health activities is required. Also regular training of health care workers in reporting adverse reactions improves both active and passive reporting.

Spontaneous reporting is a system whereby case reports of adverse drug events are voluntarily submitted by health professionals, pharmaceutical companies or consumers to the national pharmacovigilance centre. It is basically the reporting of a suspected adverse reaction on the initiative of the health professional that becomes aware of the problem, or on the patients initiative. Cohort event monitoring refers to the use of prospective, observational cohort studies of patients to whom the medicine of interest has been administered with recording of all adverse events.

Pharmacovigilance system may be run by a person or group of persons with expert knowledge of drug safety and adverse events under the independent government setup. The Central Drugs Standard Control Organization (CDSCO), Directorate General of Health Services under the aegis of Ministry of Health & Family Welfare, Government of India in collaboration with Indian Pharmacopeia commission is initiating a nation-wide pharmacovigilance programme for protecting the health of the patients by assuring drug safety. The programme shall be coordinated by the Indian Pharmacopeia commission, Ghaziabad as a National Coordinating Centre (NCC). The centre will operate under the supervision of a Steering Committee.

Admitting to affair, Petraeus resigns as CIA chief

(AP) ? The resignation of CIA Director David Petraeus has brought a sudden and unexpected end to the public career of a four-star general who led U.S. troops in Afghanistan and Iraq and was thought to be a potential candidate for president.

Petraeus admitted to an extramarital affair in tendering his resignation, which President Barack Obama accepted Friday.

Petraeus carried on the affair with his biographer Paula Broadwell, a reserve Army officer, according to several U.S. officials with knowledge of the situation. They spoke anonymously because they were not authorized to discuss publicly the investigation that led to the resignation.

The FBI discovered the relationship by monitoring Petraeus' emails, after being alerted Broadwell may have had access to his personal email account, two of the officials said.

Broadwell did not respond to voice mail or email messages seeking comment. Broadwell's biography, "All In: The Education of General David Petraeus," was written with Vernon Loeb, a Washington Post editor, and published in January.

Lawmakers from both parties joined Obama in praising Petraeus. Obama said in a statement that Petraeus had provided "extraordinary service to the United States for decades" and had given a lifetime of service that "made our country safer and stronger."

CIA Deputy Director Michael Morell will serve as acting director, Obama said. Morell was the key CIA aide in the White House to President George W. Bush during the Sept. 11, 2001, terror attacks.

"I am completely confident that the CIA will continue to thrive and carry out its essential mission," Obama said.

The resignation comes at a sensitive time. The administration and the CIA have struggled to defend security and intelligence lapses before the attack that killed the U.S. ambassador to Libya and three others. It was an issue during the presidential campaign that ended with Obama's re-election Tuesday.

The CIA has come under intense scrutiny for providing the White House and other administration officials with talking points that led them to say the Benghazi attack was a result of a film protest, not a militant terror attack. It has become clear that the CIA was aware the attack was distinct from the film protests roiling across other parts of the Muslim world.

Morell rather than Petraeus now is expected to testify at closed congressional briefings next week on the assault on the consulate in Benghazi, which occurred on the 11th anniversary of the 9/11 attacks.

Petraeus, who turned 60 on Wednesday, has been married for 38 years to Holly Petraeus, whom he met when he was a cadet at the U.S. Military Academy at West Point, N.Y. She was the daughter of the academy superintendent. They have two children, and their son led an infantry platoon in Afghanistan.

The retired general told his staffers in a statement that he was guilty of "extremely poor judgment" in engaging in the affair. "Such behavior is unacceptable, both as a husband and as the leader of an organization such as ours." He said he had offered his resignation to Obama on Thursday and the president accepted it Friday.

Administration officials said the White House was first notified about the Petraeus affair on Wednesday, the day after the election. Obama, who returned to the White House that evening after spending Election Day in Chicago, wasn't informed until Thursday morning.

For the director of the CIA, being engaged in an extramarital affair is considered a serious breach of security and a counterintelligence threat. If a foreign government had learned of the affair, the reasoning goes, Petraeus or Broadwell could have been blackmailed or otherwise compromised. Military justice considers conduct such as an extramarital affair to be possible grounds for court-martial.

Failure to resign also could create the perception for the rank and file that such behavior is acceptable.

At FBI headquarters, spokesman Paul Bresson declined to comment on the information that the affair had been discovered in the course of an investigation by the bureau.

Holly Petraeus is known for her work helping military families. She joined the new Consumer Financial Protection Bureau to set up an office dedicated to helping service members with financial issues.

Though Obama made no direct mention of Petraeus' reason for resigning, he offered his thoughts and prayers to the general and his wife, saying that Holly Petraeus had "done so much to help military families through her own work. I wish them the very best at this difficult time."

Petraeus, who became CIA director in September 2011, was known as a shrewd thinker and hard-charging competitor. His management style was recently lauded in a Newsweek article by Broadwell.

The article listed Petraeus' "rules for living." No. 5 was: "We all make mistakes. The key is to recognize them, to learn from them, and to take off the rear view mirrors ? drive on and avoid making them again."

In the preface to her book, Broadwell said she first met Petraeus in the spring of 2006. She was a graduate student at the Kennedy School of Government at Harvard; he was visiting the university to discuss his experiences in Iraq and a new counterinsurgency manual he was working on.

She had graduated from West Point with academic, fitness, and leadership honors, according to a biography posted on her publisher's website that lists authors available for speaking engagements.

The biography said she had "lived, worked, or traveled in more than 60 countries during more than 15 years of military service and work in geopolitical analysis and counterterrorism and counterinsurgency operations." It said she had spent time with the U.S. intelligence community, U.S. Special Operations Command and FBI Joint Terrorism Task Forces.

Harvard invited some students to meet with Petraeus, and Broadwell was among them because of her military background, which she wrote included being recalled to active duty three times to work on counterterrorism issues after the Sept. 11 attacks.

"I had since joined the Army Reserve and begun graduate studies with the intent of returning either to active duty or to the policy world," she wrote in the preface. She introduced herself and described her research interests. He gave her his card and offered to connect her with others working on the same issues.

"I later discovered that he was famous for this type of mentoring and networking, especially with aspiring soldier-scholars. He immediately responded to the email, inviting me to bounce ideas off him. I took full advantage of his open-door policy to seek insight and share perspectives," Broadwell wrote.

In 2008, she wrote, she was pursuing a Ph.D. in public policy and embarking on a case study of Petraeus' leadership. At one point, she said, he invited her for a run along the Potomac River with his team while he was in Washington.

"I'd earned varsity letters in cross-country and indoor and outdoor track and finished at the top of my class for athletics at West Point; I wanted to see if he could keep stride during an interview. Instead it became a test for me." He eventually increased the pace "until the talk turned to heavy breathing and we reached a 6-minute-per-mile pace. It was a signature Petraeus move. I think I passed the test, but I didn't bother to transcribe the interview."

After Obama put Petraeus in charge in Afghanistan in 2010, Broadwell decided to expand her research into an authorized biography.

Broadwell made many trips to Afghanistan, with unprecedented access to Petraeus, and also spent time with his commanders across the country. When Petraeus took the job at the CIA, she remained in close contact with him, sometimes invited to his office for events like his meeting with Hollywood actress Angelina Jolie.

With the book done, she told friends she had been concentrating on turning part of her research on Petraeus into a dissertation, to complete her doctorate.

Petraeus, in his email, told his CIA employees that he treasured his work with them "and I will always regret the circumstances that brought that work with you to an end."

The director of national intelligence, James Clapper, said Petraeus' departure represented "the loss of one of our nation's most respected public servants. From his long, illustrious Army career to his leadership at the helm of CIA, Dave has redefined what it means to serve and sacrifice for one's country."

Other CIA directors have resigned under unflattering circumstances. CIA Director Jim Woolsey left over the discovery of a KGB mole, and director John Deutch left after the revelation that he had kept classified information on his home computer.

Before Obama brought Petraeus to the CIA, he was credited with salvaging the U.S. war in Iraq.

"His inspirational leadership and his genius were directly responsible ? after years of failure ? for the success of the surge in Iraq," Sen. John McCain, R-Ariz., said Friday.

President George W. Bush sent Petraeus to Iraq in February 2007, at the peak of sectarian violence, to turn things around as head of U.S. forces. He oversaw an influx of 30,000 U.S. troops and moved troops out of big bases so they could work more closely with Iraqi forces scattered throughout Baghdad.

Petraeus' success was credited with paving the way for the eventual U.S. withdrawal.

After Iraq, Bush made Petraeus commander of U.S. Central Command, overseeing all U.S. military operations in the greater Middle East, including Afghanistan and Pakistan.

When the top U.S. commander in Afghanistan, Gen. Stanley McChrystal, was relieved of duty in June 2010 for comments in a magazine story, Obama asked Petraeus to take over in Kabul and the general quickly agreed.

In the months that followed, Petraeus helped lead the push to add more U.S. troops to that war and dramatically boost the effort to train Afghan soldiers and police.

The Senate and House intelligence committees were briefed on Petraeus' resignation only after the news was reported in the media, said a congressional staffer, speaking anonymously because the staffer was not authorized to publicly discuss the sensitive briefings.

House Homeland Security Chairman Peter King, R-N.Y., said he regretted Petraeus' resignation, calling him "one of America's most outstanding and distinguished military leaders and a true American patriot."

Senate Intelligence Committee Chairman Dianne Feinstein, D-Calif., also regretted the resignation but gave Morell high marks, too.

Morell had served as deputy director since May 2010, after holding a number of top roles, including director for the agency's analytical arm, which helps feed intelligence into the president's daily brief. He also worked as an aide to former CIA Director George Tenet.

"I wish President Obama had not accepted this resignation," Feinstein said of Petraeus, "but I understand and respect the decision."
